POLICE VETTING APPLICATION
As further protection for our junior and senior members and the organisation as a whole, Surf Lifesaving New Zealand now requires that a police check be carried out on all adults over 18 years old in direct contact with children. This is a check for “crimes against children and those of a vulnerable nature” ONLY. Any concerns are notified to the SLSNZ only and are treated with utmost confidentiality. Parents who are present for the primary purpose of care, supervision or water safety for their own child do not require Police vetting.

Once this form is completed please email to: policevetting@lifesaving.org.nz along with Evidence of Identity
Evidence of Identity
Two forms of identification must be sent in with the police vetting form, one of these must be photographic. You must also submit a photo of yourself holding their photo ID, the photo in the ID
must be visible.
a. Primary IDs include:
Passport
NZ Firearms Licence
NZ Full Birth Certificate (issued on or after 1998)
NZ Citizenship Certificate
NZ Refugee Travel Document
NZ Emergency Travel Document
NZ Certificate of Identity
b. Secondary IDs include:
NZ Driver Licence
18+ card
NZ Full Birth Certificate (issued before 1998)
Community Services Card
SuperGold Card
NZ Employee Photo Identification Card
NZ Student Identification Card
Inland Revenue number
NZ issued utility bill (issued not more than six months earlier)
NZ Teachers Registration Certificate
NZ Electoral Roll Record
International Driving Permit
Steps to Freedom Form
Current identity documents are preferred, but documents that have expired within the past five years may be accepted
